I'm sure that I'm missing something really simply but for the life of me I can't figure this out... How can I get the 5 buttons on the top the same width as the toolbar itself so that they appear similar to the two running apps.

Increase the value:
TLB Settings->Metrics->Max width of buttons with text->Small buttons

This option defines the maximum with of the buttons with text in pixels. Default value is 140.
That's close but not quite there. In order for this setting to be observed I need to also uncheck Same width for all buttons with text which makes the buttons all different widths - something I definitely don't want. I'm looking for all buttons to be the exact width of the toolbar itself.

Even if the setting you pointed out worked with Same width for all buttons with text enabled the button with the longest text still does not go quite to the edge of the toolbar. (My small button width is currently set to 500.)

There is no way to set the buttons width exactly as toolbar width. The option defines the maximum width only, the real button width depends of the button's text length.

You can increase the value: TLB Settings->Metrics->Button margins->Width
This value will always added to the left and right of the every button.

Also try to option TLB Settings->Toolbar->Default alignment. If you set this option into Centre (and if the option Same width for all buttons with text is turned on), the buttons will be centered on the toolbar.
