Only the last beta versions support PNG icons for plugins. Try with 3.2.14RC2

And png file is standard turnoff.png. Going IMHO with installation of TurnOff plugin.

Ah, I think that I found the problem. TurnOff item used Recomended for Plugins content menu there, and Appearance option was simply missed there so that the item had not assigned png file or icon.
And thank you, seems that with version 3.2.3.0 of tlb.dll the png file is supported too. So, I just suggest to adjust png file used by default with creation of new TurnOff item.

I'm trying to use just the default icon but no icon is showing up at all. This happened when I upgraded recently. I don't remember what versions I was using before. I tried adding a new instance of the Turn Off plugin but it acts just the same.
