One thing I've seemed to notice in the v3.12 beta is that if you select an icon for an item, but do not move it to the "desired default location" ( C:\Documents and Settings\%User%\Application Data\Microsoft\Internet Explorer\Quick Launch\~~images~~\icons, it often gets "lost" at a later point and needs to be reassigned.
Maybe if there was a way to have selected icons automatically moved to the "desired default location" they would stay in place.
