What version of TLB do you install?

Please try this steps:
1) Open Quick Launch
2) Right-click empty space of QL and select Open
3) Delete setup.ini and tlbdata/tlbdata.xml file

Note these file is hidden so you must enable to show hidden files in Folder Options. These files contain settings for toolbar.
