this topic will not help. TLB is different type of toolbars. It can become if you'll make changes in the registry, but this is other case.

The only thing that is correct - when TLB does not load afer restart - you have registry corrupt. To be sure that this is true try to close Quick Launch or open other standard QL toolbar and restart - your settings must be discarded. If new standard toolbar appears after restart then try to delete all plugins from TLB or even delete all buttons, maybe it crash explorer on restart and it do not save settings. Let me know if so.
