If you have TLB installed, open Start->Programs->True launch Bar->Standalone True Launch Bar
I'm thinking about something like incompatibility with one of the Windows Explorer add-ons. Running Standalone True Launch Bar will exclude the Windows Explorer.
Other thing I wanted to ask you to try. Can you create the folder somewhere on your disk. The open TLB. During click the "True Launch Bar..." in the toolbars menu hold down CTRL+SHIFT keys. TLB will ask you for toolbar folder. Select the folder you've created. This will open the clear TLB with the default settings. Now create menu and try how this clear TLB works.
Other experiment. Can you create new user? Log on as this user and try TLB.
We must find the TLB state when it works stable. And then compare this stable state with unstable. The crash reports you send me looks like the incompatibility with something. But I can't to find with what
