Maybe this is the incompatibility with any type of software you installed? Can you try to create the second user account and run TLB there?
As I see on the screenshot you have only Desktop and Quick Launch in the toolbars list. By default this menu must contain some other standard toolbars like "Address", "Links", "Windows Media Player". Usually this is because of work some of "system/registry cleaner". And usually the fixing the registry keys restore the functionality.
But if the standalone TLB don't want to work then this is the incompatibility with some other software most likely.
